I agree not to bring an aggressive pet (towards humans) for an in-person consultation with Dr. Keith. I acknowledge full responsibility for any damages or injuries my pet may cause to Dr. Keith or anyone present in the facility during our appointment. I am aware that Dr. Keith offers Telehealth appointments for aggressive pets, and I commit to scheduling such appointments if needed.

I I will pay for all my consultations at the time of booking.
I will pay for written follow-up consultations at the time of sending the written consultation request.
I understand that I must prepay for any supplements or herbal medicines available in stock or drop-shipped from our supplier. These items will not be shipped or provided to me without payment in full.

If I cancel with more than 72 hours notice, a refund will be processed to the card that payment was processed on. Refunds will be processed in 3-5 business days from cancellation.

If I cancel with less than 72 hours' notice, I will be charged 75% of the cost of my booked/paid consultation. A refund of 25% will be processed within 3-5 business days.

If I don't show up for my booked consultation, I will be charged the full amount of the of the cost of my consultation. No refund will be processed.
I understand that If I make any changes to my booked appointments, I will have to go through the online booking process again to find the next available appointment. I understand it may take months until the next appointment is available and exceptions will not be made.

Canceling on the same day is treated as a no-show, occupying a valuable appointment slot and hindering our ability to assist other animals in need. This policy is in place to ensure efficient scheduling and provide timely care to all our patients. Your understanding and cooperation in adhering to appointment guidelines greatly contribute to maintaining the quality and accessibility of our services for everyone.

I recognize that I am seeking a form of treatment for my pet that is likely to vary considerably from those offered at traditional veterinary practices. I hereby request and consent to the treatments and procedures within the scope of the practice of holistic veterinary medicine on my pet by Dr. Keith Weingardt. I understand that the intent of these modalities is to re-establish balance within the body using innate healing potential. These forms of complementary medical practice may include: nutrition, acupuncture, herbal medicine, nutritional supplements, homeopathy, energy medicine and others not listed here. I understand that herbals need to be administered according to written and/or oral instructions and will notify Dr. Weingardt immediately of any unanticipated effects associated with the administration of herbal formulas. I understand that not all patients can or will benefit from one or more of these complementary medical approaches. I accept that Dr. Weingardt may discuss, recommend, and/or prescribe other modes of care for my pet including referrals to general practitioners, boarded specialists, other alternative medical caregivers, conventional medical or surgical care. I authorize Dr. Keith Weingardt to discuss my pet's medical history and treatment protocols with my family veterinarian as needed. I also understand and accept that Dr. Keith Weingardt may decide not to offer suggested alternative medical care for my pet without further diagnostic testing or may decide not to offer such care because there is no apparent reason that it would benefit my pet. I further acknowledge that there is no guarantee as to the result of any treatment or supplement recommendation made by Dr. Weingardt. I agree to hold Dr. Keith Weingrardt harmless in the event of unforeseen incidents while my animal(s) is/are under his care. I am the legal owner or representative of the legal owner of the animal(s) I present for diagnosis and treatment. I am over the age of 18 years. I understand Dr. Keith Weingardt requires payment in full at the time services are scheduled.
